Since standard care (metal crown) costs approximately 400-500 euros, the double subsidy in many cases covers the entire cost of standard care.

The Sliding Hardship Rule

When income is only slightly above the hardship threshold, the sliding hardship rule applies. Insurance covers the out-of-pocket portion up to the excess amount — that is, the amount by which income exceeds the threshold. Example: if the out-of-pocket cost is 400 euros and income exceeds the threshold by 100 euros, insurance covers 300 euros of the out-of-pocket cost.

How to Apply for the Double Subsidy

The application is submitted together with the treatment plan (HKP) to your health insurer. Income documentation must be attached: pay slips from the last three months, for self-employed the latest tax assessment, for social benefit recipients the current benefit notice. The insurer reviews the documents and informs you whether the hardship rule applies.

Processing typically takes two to four weeks. Important: do not begin treatment until the insurer has approved the HKP with the hardship surcharge. Retroactive applications are generally not accepted.

Even with hardship status, you can choose premium dental work. The double subsidy is always paid — regardless of which material you choose. If you choose a zirconia crown instead of the standard metal crown, you receive the double subsidy of 440 euros and pay the remainder yourself. The difference must then be borne by you.
