What Is Family Insurance?

Family insurance is one of the greatest advantages of statutory health insurance: family members without their own income or with only minimal income can be covered for free through a GKV member. This applies to spouses, registered partners and children. In international comparison, this free co-insurance is a distinctive feature of the German healthcare system.

In private health insurance, there is no equivalent: every family member needs their own contract with their own premiums. For families, this can make a difference of several hundred euros monthly — a factor that is frequently underestimated in the GKV-PKV decision.

Requirements for Family Insurance

Family insurance is tied to several conditions. The spouse must not have total income exceeding EUR 505 monthly (2026); for a minijob, the limit is EUR 538. Once the spouse earns income exceeding these limits, family insurance is lost and separate insurance becomes necessary.

Additionally: if the higher-earning spouse is privately insured and their income exceeds the JAEG, children cannot be family-insured through the GKV-insured parent. This rule prevents high earners from using PKV while simultaneously benefiting from GKV family insurance. In practice, this often means that families where one partner is PKV-insured must also insure their children privately.

Further requirements: the family member must not be employed in a position subject to mandatory insurance, must not be primarily self-employed, and must be resident in Germany. There must also be no insurance exemption or exemption from mandatory insurance in place.

Children in Family Insurance

Children can be family-insured free of charge until age 18. With school attendance, training or university studies, family insurance extends to age 25. Military or voluntary service extends this period accordingly by the duration of service.

For children with disabilities who cannot support themselves, family insurance continues indefinitely. Adopted and stepchildren are also eligible, provided they live in the GKV member's household. Foster children can also be family-insured under certain conditions.

An important special rule concerns students: those over 25 or working more than 20 hours per week can no longer be family-insured and must take out student health insurance or insure themselves voluntarily. The student contribution in 2026 is approximately EUR 110 per month.

The Financial Advantage in Numbers

The savings through family insurance are substantial. A family-insured spouse would cost between EUR 200 and EUR 400 monthly in PKV, depending on age and health. Per child, it's about EUR 100 to EUR 150 monthly in PKV.

A typical four-person family (two adults, two children) saves between EUR 400 and EUR 700 per month through family insurance — that's EUR 4,800 to EUR 8,400 per year. Over the entire family phase of 20 years, the total savings amount to EUR 96,000 to EUR 168,000. This amount far exceeds the initial PKV savings and is often the decisive factor in insurance choice.

When Does Family Insurance End?

Family insurance ends automatically when the requirements are no longer met: the spouse exceeds the income limit, the child becomes too old, or takes up employment above the marginal employment threshold. Family insurance for the ex-partner also ends upon divorce.

In these cases, voluntary GKV membership becomes necessary, or the person must insure themselves independently. The insurer typically provides timely notification about the loss of family insurance. Important: report changes such as new employment or income changes promptly to your health insurer to avoid back-payments.
